DOCUMENTS REQUIRED FOR REGISTRATION WITH GWAC

GWAC मा दर्ता गर्नका लागी आवश्यक कागजातहरु:

1. Passport photo page of both Mr and Mrs – श्रीमानर श्रीमतीको पासपोर्टको फोटो भएको पाना।

2. ILE/ILR of both Mr and Mrs – श्रीमान र श्रीमतीको पासपोर्टको भिसा (आइ एल इ/आइ एल आर) को पाना।

3. BRP front and back page or print out of e-Visa settled status in the UK of both Mr and Mrs -श्रीमान र श्रीमतीको बी. आर. पि. कार्डको अगाडि र पछाडिको पाना अथवा युके आवासीय ई-भिसाको प्रिन्ट कपी।

4. All pages of AF B108, Service Certificate (Lal Book) -लाल बुकको सबै पानाहरु।

5. Completed and signed Consent Form -भरेको र हस्ताक्षर गरेको सहमति फारम।

6. A proof of address if available -यदि भएमा ठेगाना प्रमणित गर्ने कुनै एक कागजात।

GWAC CONSENT FORM – ONLINE

The Gurkha Welfare Advice Centres (GWACs) hold personal data in the form of documents and information about you to be able to provide you with assistance when working with Service charities, Government agencies and other authorities on your behalf. GWACs will use the information to contact you and to discuss your case with relevant charities and organisations as authorized by you.

GWACs abide by the General Data Protection Regulation (GDPR) and will ensure that any data held is relevant, accurate and up to date. It will not be kept for longer than necessary. It will be kept secure and appropriately destroyed when it is no longer required. Be assured that we will take appropriate measures to prevent unauthorised processing, selling or loss of your data.

However, we need your formal consent for us to use your data to conduct our business.

By completing this form, you give explicit consent to GWACs to confirm that you have provided us with your correct details and to process your data as stated below:

• I/We give my/our consent to GWACs to record my/our personal data and to keep my/our discussion notes.

• I/We agree that the Department for Work and Pensions can use the information provided to assess whether I have a National Insurance number and if not, to allocate the number to me if I am eligible.

• I/We authorise any GWT or MOD employed GWACs staffs to take up enquiries and receive information on my/our behalf including confidential medical details where appropriate. Those staffs are authorised to disclose and provide details of my/our accounts, and to work on my/our behalf in relation to my/our social security benefits, welfare needs and documentations and to be notified of the outcome.
